Communities in Action for Asthma Friendly-Environments

The Communities in Action for Asthma-Friendly Environments Change Package is designed to help community-based asthma management programs learn about five key change concepts that drive program effectiveness and improve health outcomes for people with asthma. Community-based asthma management programs that use this Change Package will find specific, replicable, and field-tested strategies they can follow to improve asthma health outcomes in their communities.

This Change Package is a work in progress. Version 2.0 of the Change Package was debuted at the 2007 National Asthma Forum and we continue to work with the Communities in Action for Asthma-Friendly Environments Network to refine and improve the Change Package.
