Welcome to the online registration for EPA’s Communities in Action for Asthma-Friendly Environments National Forum. This is a secure Web site operated for the U.S. Environmental Protection Agency (EPA) by its contractor, The Cadmus Group, Inc. The information you submit will be seen only by EPA staff and EPA’s contractors.

Getting Started

In order to use the registration form, you must use a Web browser that accepts cookies. You must also have Javascript enabled. If for any reason you are having trouble using this online registration site, please send an email to info@epaasthmaforum.com.

You will be required to create an account by entering an e-mail address and a password. Having an account allows you to partially complete the registration form, save it, and return to it later. Once you have created an account, you will be able to modify either the password or the e-mail address. The e-mail will be used to send you verification of your registration and payment and enable you to retrieve a forgotten password. Passwords must be at least 5 characters long. Please note that the e-mail address and contact information you provide will be included in the attendee rosters. If you do not wish to have this information to be included in the roster, please e-mail info@epaasthmaforum.com.

If you forget your password, you can retrieve it by clicking the Retrieve Password link on the Log In page.

When you have finished registering for the National Forum, you can end your session by clicking the link labeled "Log Off" on the left navigation menu, closing your browser, or navigating to a different site. If you close your browser or navigate to a new site, be sure you save any new entries by clicking "Save" before exiting this Web site.

If you do not complete your registration form in one sitting, you may return to it later. Be sure to click "Save" before leaving the site.
